Dental Plan options include HMO plans for groups, families and individuals starting as low as $6.95 a month. Dental Indemnity and Preferred Provider Organizations (PPO) plans are available for groups of 2 or more. Rates for indemnity and PPO plans vary based upon group size, location and desired levels of benefits.
